Birth control for acne

Editorially reviewed on October 29, 2026. [TODO: medical reviewer]

Quick answer

Combined pills can improve acne, and cyproterone acetate pills (Diane-35, Althea, Cybelle) are approved for moderate to severe hormone-related acne, usually after creams and antibiotics have not worked. Acne typically takes a few cycles to improve and can come back when you stop. Progestin-only methods do not reliably help acne and can sometimes make it worse.

What the evidence says

Estrogen-containing pills lower the hormones that drive oily skin and breakouts. The European Medicines Agency reviewed cyproterone acetate with ethinylestradiol in 2013 and concluded its benefits outweigh the risks for moderate to severe androgen-related acne and excess hair, ideally after topical treatment and oral antibiotics have failed [src]. It should not be taken together with another hormonal contraceptive, because that raises the risk of blood clots [src].

Method families to discuss with your doctor

  • Cyproterone acetate pills: Diane-35 (cyproterone acetate + ethinylestradiol), Althea and Cybelle (both cyproterone acetate + ethinylestradiol) have the same formula. Cybelle starts at ₱525, Althea at ₱537, Diane-35 at ₱695.75. See same formula, lower price.
  • Drospirenone pills: Yaz (drospirenone + ethinylestradiol) and its twins Lizelle and Ermina, or Yasmin and its twins Liza and Erin. Filipinas with PCOS often compare these with Diane-35 [src].
  • Other combined pills may also help, but less is known about each one for acne specifically.

What to expect

Acne usually improves over several cycles, and an early breakout in the first month is common [src]. Give it at least 3 months before judging. Acne often returns after stopping. Combined pills are not suitable for everyone: if you smoke and are 35 or older, have migraine with aura, or have had a clot, ask about other options [src]. A dermatologist can also help with creams and other treatments.

Acne with irregular periods or excess hair?

That can be a sign of PCOS. See birth control and PCOS.

Frequently asked questions

Is Cybelle the same as Diane-35?

Yes. Cybelle, Althea and Diane-35 all contain cyproterone acetate 2 mg with ethinylestradiol 35 mcg. They differ in price and brand.

Will my acne come back if I stop the pill?

It often does, because the pill controls the hormones rather than changing your skin permanently.

Can I take Diane-35 with another birth control pill?

No. It should not be combined with another hormonal contraceptive because of the higher risk of blood clots.
